Description (Service)
The Hospital Elder Life Program (HELP) is an innovative delirium-prevention program designed to improve the hospital experience of older patients.
The HELP program aims to:
  • Maintain the cognitive & physical functioning of patients throughout hospitalization
  • Allow patients to be discharged from the hospital as independently as possible
  • Prevent unplanned readmissions.

    The HELP team is run by an Elder Life Specialist and a team of volunteers, and offers the following services:
  • Daily visiting, meal support, early mobilization,therapeutic engagement, vision and hearing support, and sleep help.

Eligibility Must be an in-patient on specific units at PRHC and at-risk for delirium. Must be at least 65 years of age.
