Eligibility | Must be an in-patient on specific units at PRHC and at-risk for delirium. Must be at least 65 years of age. Discharge not anticipated within 48 hours, be able to participate in the program, not on droplet or airborne isolation precautions(contact isolation only) and have at least one of the following: Cognitive impairment, changes in mobility, vision/hearing impairment an/or dehydration/malnutrition risk. |
Application | Referral Required; Referral from a health care or social service provider. Patients can make a referral to this program by speaking to their nurse or allied health provider |
