Help aboriginal patients with cancer access health care support and services at home- co-ordinate traditional and non-traditional community resources and healing
- counselling and support before, during and after clinical appointments
- liaison/support between the care team, patients and families
- advocacy on behalf of patients
- assistance with care planning
- advocacy for end of life care focused on the individual's goals and wishes
