Description (Service) Help aboriginal patients with cancer access health care support and services at home
  • co-ordinate traditional and non-traditional community resources and healing
  • counselling and support before, during and after clinical appointments
  • liaison/support between the care team, patients and families
  • advocacy on behalf of patients
  • assistance with care planning
  • advocacy for end of life care focused on the individual's goals and wishes
