Description (Service) Nurse Practitioner-led clinic provides accessible and comprehensive primary health care services for people who do not have a family doctor or nurse-practitioner
  • not a walk-in clinic - clients must register prior to service and become a rostered patient to recieve services.
Nurse Practitioners, Registered Nurses, Dietitian and Social Workers also onsite

Eligibility
  • Must NOT already have a family doctor or Nurse Practitioner
  • Must be a rostered patient with clinic (except for use of laundry/dryer and showers by individuals who lack access due to income or homelessness)
Application
  • Registered patients can call for an appointment
  • Complete a referral form (currently have a waitlist)
Languages English; Interpretive Services - can be provided when required
Fees None - all services covered
