Developed and implement customized care plans:

Personal care support, grooming, transferring
Medication monitor
Respite care
Hospital to Home Transition
Caregiver relief
Palliative care
Medication administration
Advocacy and coordination of medical and social services
Homemaking services including meal preparation, laundry, errands and light housekeeping services
assistance with activities of daily living including accompaniment and transportation to appointments
Companionship
Chair Yoga
Dementia care; registered with Alzheimer's Society as a Best Friends Approach Trainer
Assistance for those recuperating from motor vehicle accidents or post-surgery
Offer some services free of charge depending on the care that is needed. (i.e. grocery run or pharmacy is at no charge.)
