Description (Service)

Connects people with health care services at home, at school or in the community, including community support services, supported living programs and long-term care options.

  • Provides information and direct access to health care providers and services to help people come home from the hospital sooner or live independently at home longer
  • Assists people to find their way through Ontario's health care system, from school children who have special health needs to seniors who need health services at home or access to a long-term care home
  • Services may include nursing, physiotherapy, occupational therapy, speech-language therapy, social work, dietetics, personal support and medical supplies and equipment
  • Specialized programs include acquired brain injury, convalescent care, school health support services and palliative care
  • Community Education and Outreach provides presentations and information displays to community organizations
